Endeavor Plus Terms and Conditions

Enrollment Agreement:

By agreeing to this Enrollment Agreement, I request that the School reserve a place for my child in the program(s) noted. Upon receipt of the Enrollment Agreement by the School, I understand my obligation to pay the tuition and fees for all programs noted above and that no portion of fees or tuition paid will be refunded or canceled in the event of absence, illness, failure to attend or withdrawal (without meeting the withdrawal notification period) of the above student.

Tuition and Fee Payments:

I understand that I am responsible for payment of the program charges that will be billed to me. We reserve the right to increase tuition prices to meet economic changes when necessary.

I understand that upon the return of this Enrollment Agreement, my account will be charged the Registration Fee and any deposits that may be required to secure my child’s placement.

Policies:

Behavior Policy

• Students participating must adhere to the rules set forth by the instructors at all times.

Liability Waiver

• My child(ren) has no limitations, injuries, or conditions that would affect said child’s ability to participate safely in this program.

• In consideration of being permitted to attend Endeavor Plus Classes (DanceStars, Active Athletes, STEM, ART and others) on behalf of myself, my family, my heirs, and my assigns, I hereby release Endeavor Plus , LLC, its agents, and its employees from liability for injury, loss, or death to the registered participant while using any facility or equipment or in any way associated with participating in the activities Endeavor Plus offers now or in the future.

• I have read the preceding information and do not need further clarification. I know, understand, and appreciate the risks associated with participation in the activity and I am voluntarily enrolling my child in the activity. I acknowledge that the participant is assuming all of the inherent risks of the activity. I further understand that in the event of a medical emergency, Endeavor Plus will call EMS to render assistance and that I will be financially responsible for any expenses involved.

Social Media and Photo Release

• I grant permission for my child's image to appear on the Endeavor Plus or School Facebook/Twitter accounts as well as webpage.

• I agree to allow Endeavor Plus to take photographs and video of my child and to use on its websites and social media pages including letsmove.endeavorschools.com, Instagram, Twitter, Facebook and promotional materials without compensation. I understand that my child's identity will be kept confidential.

Make up Sessions and Refunds:

• Refunds/make-up sessions will not be made for non-attendance for a scheduled session, including if your child chooses not to take part in the session for any reason.

• Refunds/make-up sessions will not be made for school related absences, such as field trips, photo days, or other similar instances. (Please note, we make every effort to work within the school’s schedule to avoid such incidents as much as possible.)

Withdrawal

• Withdrawal requires 30 days (or more) notice via email. Verbal notice is not a valid form of cancellation. Contact the school admin with the notice or any questions.

• Withdrawals are not prorated or refunded. Upon enactment of Withdrawal, auto-rebilling ceases.

Class Meetings and Schedule

Classes are scheduled to take place once a week on a given day.  If the school is closed for any reason (such as holidays) the sessions will not be made up.
